1. I, the pleasure craft operator, confirm that there are the appropriate number and sizes of approved personal floatation devices in the boat for the number of people on board and they must be worn and that by not abiding to these rules I may be subject to ticketing and a fine. ________ 2. I, the pleasure craft operator, am aware that it is illegal to operate a pleasure craft, or to permit others to do so, when under the influence of alcohol, narcotics or barbiturates. ________ 3. I. The pleasure craft operator, will only allow people who have completed and signed a rental boat safety checklist, valid for the duration of this rental or have other proof of competency, to operate this craft. ________ 4. I, the operator, have been shown how to use, and know the location of safety equipment required under the small vessels regulations. ________ 5. I, the operator, understand that pleasure craft fitted with motors must be kept clear of non-powered pleasure craft. ________ 6. I, the operator, when involved in an accident must stop, offer assistance and give my identity. ________ 7. I am aware of how to responsibly operate this craft. ________ 8. I, the operator will not exceed the number of persons recommended for the craft . ________ 9. I have been shown the proper procedure for starting and shutting off engine and I will ensure that no person shall be in the vicinity during these operations. ________ 10. I have been shown and understand the operation of the throttle and gear shift lever. ________ 11. I know where and how to operate the ignition cut-off switch. ________ 12. I am aware of how to depart and approach the dock in a safe and proper manner. ________ 13. I am aware of how to respond, capsizing and re-boarding. ________ 14. I am aware of local hazards and regulations. ________ 15. I shall maintain a proper lookout at all times for other boats, seaplane, hazards and changes in weather conditions which can quickly create dangerous situations. ________
